IUI or timed intercourse: what is the difference?

In timed intercourse, the couple tries at home on the days when ovulation is expected, often after scans or medicines. In IUI, washed sperm is placed directly inside the womb at that time. IUI puts more moving sperm closer to the egg, but both still need an open tube and an egg that is released.

In this video our fertility specialist explains the difference between IUI and natural intercourse.

The difference between IUI and natural sex

When is timed intercourse advised?

When ovulation is irregular but the semen report and the tubes are normal, the first step is often medicine to help an egg grow, with scans and timed intercourse at home. It is simple, private and needs no procedure.

When is IUI advised?

Neither timed intercourse nor IUI helps when both tubes are blocked or the sperm count is very low; then IVF is usually advised.

What timed intercourse involves

When cycles are being followed, a scan is done a few days before the expected ovulation to see the growing follicle. When it is close to mature, you are told which days to try, and sometimes a trigger injection is given so ovulation happens at a known time. Intercourse is usually advised on the day of the trigger and the next one or two days. You do not need to try every day of the month; a few well-timed days around ovulation are enough.

What an IUI cycle involves

The same scans and, often, the same trigger are used. On the planned day, the husband gives a semen sample, the lab washes it, and the washed sperm is placed in the womb with a soft, thin tube. It takes a few minutes and you can go home soon after. Couples can still have intercourse around the IUI day.

Questions to ask before choosing

How long to try each step

The plan usually sets a limited number of cycles for each step, for example a few timed cycles and then a few IUI cycles, rather than repeating the same treatment for a long time. Age matters here: after the mid-thirties, the plan moves forward sooner, because time affects the eggs. If several well-timed cycles have not worked, it is reasonable to look at the next step with your doctor.

Keeping it relaxed at home

Timed intercourse can make sex feel like a task. It helps to keep the rest of the month relaxed, to talk openly as a couple, and to tell your doctor if timing is causing difficulty. If intercourse on the fertile days is not possible, IUI is one way around it.

The role of the semen report

Before choosing between the two, a semen analysis is done, and it is often repeated if one result is low, because samples vary. When the count and movement are normal, timed intercourse is usually enough to start with. When they are mildly low, washing the sample and placing it in the womb through IUI puts more of the moving sperm where they are needed. Read our guide to the semen analysis report to understand each value.

Cost, time and privacy

Timed intercourse needs only scans and, sometimes, tablets or a trigger, and everything else happens at home. IUI adds a lab step and a short visit on the day. Many couples start with timed cycles and move to IUI when there is a clear reason.

Questions about IUI and timed intercourse

Is IUI better than trying naturally?

It depends on the cause. IUI helps some couples, for example with a mild sperm problem, while timed intercourse is often the first step when only ovulation is irregular.

Should we have intercourse around the IUI day?

Usually yes, unless your doctor says otherwise. Intercourse around the IUI day does not reduce its chances.

Do we need scans for timed intercourse?

Scans and sometimes a trigger injection help pinpoint ovulation, which makes the timing more accurate.

When should we move from timed intercourse to IUI?

If a few well-timed cycles have not led to pregnancy, or if there is a mild sperm problem, your doctor may suggest IUI.

Can IUI work if the tubes are blocked?

No. Both IUI and intercourse need at least one open tube. If both are blocked, IVF is usually advised.

Which days should we try in timed intercourse?

Usually on the day of the trigger injection and the next one or two days, or as your doctor advises from the scan.
